Chemical description
Benzodiazepine dervative with calcium antagonistic action, metabolized to desmetyl and desacetyl diltiazem. Substrate of CYPs 2C8,2C9, 2D6, 3A4, 3A5, 3A7. Inhibitor of 2C8, 2C9, 2D6. Potent mechanism-based inhibitor of 3A4. Andersson, patient report (n=3): tolerated. James and Hift. Porphyrias. Br J Anaesth 2000; 85, 143-53: use with care. South African list: use with care.
